Required Forms

All riders and participants are required to have completed forms. This insures that everyone is familiar with the policies and procedures of our center. Should you have questions, please do not hesitate to call. There are requirements that legal guardians sign the policies in front of RCTRC staff - please bring the documents to your first

	Equine Assisted Activities Therapy Packet

	Hippotherapy Therapy packet - Participants must obtain a physician's referral for Occupational Therapy prior to the first visit. Their diagnosis needs to be on the referral.
